What you need to start
If you already practice private medicine in Spain, you most likely have everything.
Active medical license
Active license number with any official medical board in Spain.
Professional Liability Insurance
Up-to-date professional liability policy. We can help if you need to get one.
Self-employed status (RETA) or company
Registered as autónomo or working through a medical company. Required for invoicing.
Smartphone with data
iPhone or Android to receive requests and share location during the visit.
Bilingual ES/EN
Ability to provide care in English (tourists) plus Spanish. Other languages are a plus.
Basic medical bag
Sphygmomanometer, stethoscope, otoscope and exam supplies. No heavy equipment.

Frequently asked questions
What is OnCall's commission per visit?
10% during your first year after activation, 15% from year 2 onwards. The commission is all-inclusive — it covers VAT and Stripe fees. You pay nothing else. On a €150 visit you net €135 to your bank in year 1 (€127.50 from year 2).
When do I get paid?
Stripe Connect automatically transfers your share (visit total minus commission) to your IBAN within 24-48 hours after you mark the visit as completed. No monthly aggregation, no retention on our side.
Do I have to accept every request?
No. You only receive requests when “Available now” is on. The patient picks you before paying — if you don't want to go out, you don't appear. Once you accept, the visit can't be cancelled without cost except for legitimate cause.
Is there exclusivity?
None. Keep working in the public system, in another private clinic, on another platform — wherever you want. OnCall doesn't require minimum dedication or stop you from seeing patients off-app.
Who is responsible for clinical care?
You, as the licensed doctor, are the independent data controller for clinical treatment during the visit (Art. 9.2.h GDPR). OnCall Clinic is a technology intermediary — it doesn't handle health data or interfere with clinical decisions.
What's the geographic coverage?
Ibiza is live. We're recruiting in Madrid, Barcelona, Valencia, Sevilla, Málaga, Bilbao, Palma, Alicante, San Sebastián and Tenerife (10 cities total). Each city goes live when we reach a critical mass of doctors.
What if there's an emergency or complication during the visit?
As in any private medical act, you follow standard clinical protocols and, if needed, escalate to 112 / hospital emergency. Your professional liability insurance covers the medical act. OnCall doesn't interfere with clinical decisions.
How do you block my schedule while I'm on another consultation?
While your availability is on and you haven't marked a visit as completed, OnCall does not auto-assign a second consultation to you. You can also toggle availability off manually from the dashboard if you need a break between visits. Assignment is always explicit confirmation on your side — never auto-accept.
